I would tell her - most people know about IBS in some way and chances are she'll be sympathetic. Especially since you're taking time off and the lateness thing. I always find a bit of understanding goes a long way. I told my boss and he was really good about it. Since I'm a teacher and couldn't leave the class for an attack, he arranged that another teacher could "cover" for me on a moment's notice. That really took the pressure off.
